Informal reading Assessment

Graded Word List Assessment
This student read the 4th graded word list with one error. He mispronounced frightened as “frighten” he left out the “ed” ending. Otherwise, he completed the list with 95% accuracy and would be reading at an Independent level. In reading the 5th graded word list with one error being he mispronounced establish as “established” adding the “ed” ending to the word. So, he completed this with 95% accuracy and would be reading at an independent level. When he read the 6th graded word list he mispronounced the word apparatus as “apartus”, the word temperate was mispronounced as “temperate” and the world deliberate was pronounced “dayliberate” he completed this list with 85% accuracy and he would be reading at a frustration level.
As the teacher, I would consider him to be reading at an independent level at 4th grade and seems to also be able to read independently at a 5th grade level. The difficulty and frustration did not show up until the 6th grade level leading me to believe he is doing very well with his reading in the 4th grade level.
